Medicare Advantage Plans 2027: What’s Changing

Medicare Advantage plans for 2027 are undergoing some of the most significant changes in years. If you’re currently enrolled — or considering switching during AEP 2026 — understanding what’s changing is critical to making the right decision. The Annual Enrollment Period runs October 15 to December 7, 2026.

What Is Medicare Advantage?

Medicare Advantage (Part C) is an alternative to Original Medicare offered by private insurance companies approved by CMS. These plans bundle Part A (hospital), Part B (medical), and usually Part D (prescription drugs) into a single plan, often with extra benefits like dental, vision, hearing, and over-the-counter allowances.

As of 2025, more than 33 million Americans — nearly half of all Medicare enrollees — are in Medicare Advantage plans.

Key Changes to Medicare Advantage in 2027

1. Supplemental Benefit Adjustments

Many Medicare Advantage plans competed aggressively in recent years with generous extras — OTC cards worth $500-1,000/year, dental allowances, transportation credits. In 2027, many plans are pulling back due to CMS payment rate adjustments. If you chose your plan primarily for its OTC allowance or dental coverage, check your 2027 Summary of Benefits carefully. The benefit you counted on may be reduced or eliminated.

2. Provider Network Changes

Networks are renegotiated annually. Even if your plan seems unchanged, your specific doctors may have left the network for 2027. Before AEP closes December 7, call your doctors directly to verify they’re in-network for next year. Don’t rely on online directories alone — they’re often outdated.

3. Star Rating Changes

CMS releases updated star ratings each fall. Plans with 5 stars can enroll new members year-round. Plans with 3 stars or below have documented quality concerns. If your plan’s star rating dropped significantly for 2027, seriously consider switching. Check ratings at medicare.gov/plan-compare.

4. Premium and Cost-Sharing Changes

Some plans that were $0/month are introducing premiums for 2027. Others are increasing deductibles and copays. Watch for: premium increases, deductible changes, higher copays, and MOOP changes. Use the Medicare Plan Finder to calculate your estimated total annual cost for any plan you’re considering.

5. Part D Drug Coverage Changes

The Inflation Reduction Act’s $2,000 out-of-pocket cap on Part D drug costs continues in 2027. Drug formularies also change annually — medications that were Tier 2 (preferred generic) in 2026 may be Tier 3 or 4 in 2027. Run your specific medications through any plan you’re considering before enrolling.

6. Plan Exits and Market Consolidation

Some Medicare Advantage plans are exiting certain service areas for 2027. If your plan is exiting your market, you’ll receive notice and have a Special Enrollment Period to find new coverage. Don’t ignore these notices — failure to act means defaulting to Original Medicare without a Supplement plan.

Who Should Consider Switching Medicare Advantage Plans in 2027?

Consider switching if:

  • Your plan’s star rating dropped below 3.5 stars
  • Your premium is increasing by more than $30/month
  • A doctor you rely on left the network
  • Your medications moved to a higher cost tier
  • Dental or OTC benefits were significantly reduced
  • You had a new diagnosis and need specialist access
